In consideration of my, or my child or ward’s participation in a Proskills Lacrosse, Inc. d/b/a Proskills Lacrosse interaction, I, am fully aware of and accept the risks, including the risk of catastrophic injury, paralysis, and even death, as well as other damages and losses, associated with participation in a lacrosse-related event. I further agree, on behalf of myself, my heirs, and personal representatives, that Proskills Lacrosse, Proskills Lacrosse members, leagues, teams, host organizations, contractors, and/or sponsors of recognized or sponsored events, together with their respective affiliates, employees, agents, officers, directors, volunteers, coach, officials, shall not be liable for any injury, loss of life, or other loss or damage occurring as a result of my participation in any such Proskills Lacrosse recognized or sanctioned event.
